Insurance/Payments
While we accept most insurance plans, please check with your insurance company to make sure the doctor is on your plan. As a courtesy, an insurance claim will be submitted for your visit. In order for this to be done, you must present your current insurance card at each visit. If you do not have your insurance card you will be expected to pay for your visit in full or you may reschedule
